REQUEST FOR PROPOSAL (RFP)
SCREENING AND DIAGNOSTIC SERVICES FOR THE
GUAM BREAST AND CERVICAL CANCER EARLY DETECTION PROGRAM

The Guam Department of Public Health and Social Services (DPHSS), Guam Breast and Cervical Cancer Early Detection Program (GBCCEDP) is requesting proposals from qualified medical facility(ies) or individual(s) for Screening and Diagnostic Services for the GBCCEDP.
RFP Specifications are available for public viewing, and may be picked up at the GBCCEDP office, Rm. 160, first floor, Central Public Health Facility, Mangilao, Guam beginning February 9, 2018 from 9:00 AM – 4:30 PM. In keeping with the mandates of Public Law 31-228 §5220(b), prospective respondents must register contact information to ensure notices regarding any changes or updates are forwarded. RFP Program Specifications and Registration Form may be downloaded at DPHSS website (www.dphss.guam.gov). The completed Registration Form should be returned to GBCCEDP by fax to 671-734-7626 or email to elizabeth.guerrero@dphss.guam.gov.
This procurement is 100% federally funded by the U.S. Department of Health and Human Services Centers for Disease Control and Prevention, Cooperative Agreement Grant No. 1 NU58DP006269-01-00.
Interested offerors shall submit proposals to: DPHSS Attention: GBCCEDP, Rm. 160, 123 Chalan Kareta, Mangilao, Guam 96913-6304 no later than 4:00 PM on March 23, 2018. Fax, email, oral and late submissions will not be accepted.
