RFP/DPHSS-2018-05 REQUEST FOR PROPOSAL TOBACCO FREE GUAM QUITLINE: TOBACCO & NICOTINE CESSATION PHONE AND WEBSITE SERVICES The Guam Department of Public Health and Social Services (DPHSS), Tobacco Prevention and Control Program (TPCP) is requesting proposals from qualified offerors to provide an island-wide telephone and web-based tobacco and nicotine cessation service, to be known as the Tobacco Free Guam Quitline. RFP Specifications are available for public viewing, and may be picked up at the TPCP office, Rm. 230, Central Public Health Facility, Mangilao, Guam beginning March 28, 2018 from 9:00AM – 4:00PM, or may be downloaded from the DPHSS website at www.dphss.guam.gov. In keeping with the mandates of Public Law 31-228 §5220(b), prospective respondents must register contact information to ensure notices regarding any changes or updates are forwarded. The Registration Form may be found in Appendix IV of the RFP. The completed Registration Form should be returned to TPCP by fax to 671-735-7500, or email to elizabeth.guerrero@dphss.guam.gov. This procurement is 100% federally funded by the U.S. Department of Health and Human Services Centers for Disease Control and Prevention, Cooperative Agreement Grant Nos. 6 NU58DP005057-05-00 and 5 NU58DP005352-04-00.
