WIC Program Vendor Open Enrollment

The WIC Program of the Department of Public Health and Social Services (DPHSS) is announcing the open enrollment period for all interested retail food vendors (including military food vendors) to apply to become authorized WIC food vendors for a 3-year period from October 1, 2012 to September 30, 2015.  The WIC Program will select a limited number of retail food vendors.  The open enrollment period begins 8:00 am, Monday, July 2, 2012.

Retail food vendors interested in applying can pick up a vendor application packet at the WIC Program Administration office located at 15-6100 Mariner Avenue, Tiyan, Monday through Friday, from 8:00 a.m. to 5:00 p.m., commencing Monday, July 2, 2012. 

Completed vendor applications must be submitted to the WIC Program Administration office in Tiyan, no later than 5:00 p.m. Friday, August 3, 2012.  Fax transmittals are not accepted.  No applications will be accepted after the deadline.  For additional information, contact the WIC Program Vendor Management office at 475-0291/0300, or fax at 477-7945.
