About the Program
The Guam Diabetes Prevention and Control Program (DPCP) is operated by the Chronic Disease Prevention and Control Section, Bureau of Community Health Services, Division of Public Health in the Guam Department of Public Health and Social Services.
 

Funding
The Guam DPCP is 100% federally funded and implemented under a cooperative agreement between the Centers for Disease Control and Prevention (CDC), through its Division of Diabetes Translation, and DPHSS.

 
Program Purpose
Improve access to and quality of care for all and reach communities most impacted by the burden of diabetes thus reducing the mortality, morbidity, and costs associated with uncontrolled diabetes and its related health complications.
 
Target Group
All persons residing on Guam especially those who are uninsured and under-insured.
 
Services Provided
The Guam DPCP is a CORE prevention and control program thus no direct services are provided, however, the Guam DPCPs networking abilities allow persons with diabetes and their families to participate in the following islandwide activities:
• Community health screenings
• Work-site wellness programs
• Pacific Diabetes Today Curriculum Training
• Educational intervention and counseling
• Diabetes Health Disparities Collaborative
• Community support group
• Advocacy and policy making
