The Department of Public Health and Social Services (DPHSS), Division of Senior Citizens (DSC) will celebrate our island’s Centenarians throughout the year, on the month of their birthday, and culminating in May 2022 with Senior Citizens Month.  A Centenarian is any senior citizen who is or will be 100 years of age or older between January 1, 2022 and December 31, 2022. In addition, for the purpose of this program, and to be eligible, the Centenarian’s primary residence must be in Guam.

To assist DSC in recognizing Guam’s Centenarians, the public is encouraged to inform a Centenarian or the Centenarian’s family or caregiver about this hallmark event and how to obtain the information to submit to DSC.  Biographical and Consent for Release of Information forms must be filled out for Centenarian.   

The Biographical and Consent for Release of Information forms can be found via the DPHSS website at www.dphss.guam.gov; or requested via email to be sent directly to:
